Dysmenorrhea is estimated to effect upwards of 70% of women during their lifetime and “heavy menstrual periods” is one of the most frequently given reasons for hysterectomy in the United States.  Despite the dramatic growth of dysmenorrhea treatment  with non-steroidal anti-inflammatory agents (NSADs) in the 1980s, young women continue to go undiagnosed and under treated. The diagnosis and management of heavy menstrual bleeding has even poorer general understanding because of its more complicated etiologies. With a working knowledge of the underlying pathophysiology, all clinicians should be able to establish both the diagnosis and a credible treatment plan that will result in a significant chance of clinical improvement for most patients. L'hystérectomie est une technique peu pratiquée chez le bovin. Décrite comme
risquée, les taux de survie sont plutôt satisfaisants comme le démontre le sondage
effectué auprès de vétérinaires.
Cette opération est la chirurgie de la dernière chance puisque le plus souvent,
l’alternative semble l’euthanasie. Elle a pour but d’enlever un utérus trop abîmé en
sauvant la vie de la vache. Cependant, bien que perdant ses qualités reproductives,
il a été décrit qu’une vache laitière ovariectomisée en plus d’être hystérectomisée peut
produire du lait pendant encore deux ans avant d’être réformée.
Que ce soit par laparotomie ou par voie externe, cette opération est délicate puisque
le risque d'hémorragie est important. Très souvent, la vache est en état de choc ; ce
qui augmente les risques d'échecs ; il faudra donc traiter cet état en priorité.
Plusieurs approches sont décrites pour cette chirurgie :
- Par voie externe (se fait le plus souvent en cas de prolapsus utérin).
- Par laparotomie dans le flanc gauche sur vache debout.
- Par laparotomie paramédiane sur vache couchée.
- L’hystérectomie partielle.
Pour chaque approche, il est décrit plusieurs techniques et le chirurgien peut décider
de suturer “en masse” ou ligaturer spécifiquement les vaisseaux. Il peut aussi choisir
de laisser le col de l’utérus ou de l’amputer.
Exceptée pour la voie externe où la technique fermée est préférée à l'unanimité, la
littérature actuelle ne permet pas de départager les autres techniques.
